
OUTREACH SERVICES
A Public Health Nurse, (PHN) oversees the outreach program that emphasizes
prevention of illness, promotion and maintenance of health. The Community Health
Representatives (CHR’s) are supervised by the PHN, and identify individual,
family, and community needs. CHR’s assist with creating health goals and plan
programs to meet health needs. Community health programs and health education is
provided for the Native American Community. In coordination with the Pit River
Tribal Office, Environmental Health Services are provided to Native Americans
residing within the program service area. Services include, but are not limited
to: environmental education and information, injury prevention, disease and
infection control, home water quality management, and assistance in the
development of water, waste disposal, and other essential sanitation facilities.
