Vaccine Assessment Survey

PRHS is participating in the Vaccine Assessment grant affiliated with CRIHB. A survey is being offered for participants over age 18 and a $25 gift card given when the completed survey is turned in. Surveys will be available at our clinic in Burney. We hope to assess the feedback on areas that may improve vaccine services in our community.
